Diamond ground samples

i just went to order some samples. i have noticed that some of you guys have ordered several of these, often of all different types/sizes. do you fill out a form for each of these ? or just ask for what you want in "comments" ?

i'm figuring what i want is

1.5% lanthanated
3/32, regular rrind, and polished grind
1/16, regular and polished

how should i fill out the form ?

I filled out their form two times and didn't recieve one tunsgsten. Maybe I'm not doing it right.
